About this Consult:
Anna is experiencing non-specific hamstring, pelvic floor and sacroiliac joint region pain.
It all started in her early pregnancy as a hamstring strain with running.
It still persists 2 1/2 years later after cesarean birth on her left side.
Anna also has Crohn’s disease.
This was severely flared during her pregnancy leading to fecal incontinence and a fistula with complications.
Because of this she feels she has lots of muscle clenching, but also reports an inability to activate her pelvic floor.
Anna has already explored manual therapy and pelvic floor muscle therawand self-release strategies with other therapists. She reports that these offer only temporary relief at best.
Anna was told to do some breathing and yoga to down-regulate but it only made the pain worse.
At this stage the constant pain is impacting Anna’s ability to run and is limiting her ability to level up with her mountain biking.
Anna also experiences stress urinary incontinence with two-foot jumping.
Anna is very frustrated by her symptoms and fears there may be an underlying cause that she hasn’t addressed.
She also says people tell her she’s a hypochondriac.
Anna thinks she may have a disc bulge or something along her nerve pathways, but exercises for this or along the hip and leg have only made things worse.
Anna finds her pain is easily aggravated, and single leg activities are the worst.
Anna is frustrated that her pain persists and that she hasn’t been able to fix herself as an Ortho and Women’s Health Physiotherapist.
