About this Consult:
Erica is having stress urinary incontinence during CrossFit classes, which she commenced 6 months ago. Specifically box jumps, running, heavy lifts and skipping. Erica is a mother of three children aged between 2 and 7 years old.
Erica experienced grade 4 tearing with her first birth vaginal birth, and mild tearing with the other two vaginal births.
She has a diagnosed grade 2 cystocele (bladder prolapse).
Erica had a three-finger wide diastasis and sacroiliac joint pain, which have decreased since starting CrossFit.
However, her umbilical hernia seems to be getting worse and she has strange lower abdominal cramping with heavy lifting.
Erica has found that she can improve her bladder leaking if she changes position during box jumping.
Erica finds that with running the bladder leaking fluctuates.
With double unders (skipping), Erica has uncontrollable leaking after 20 jumps no matter what position or pelvic floor muscle squeezing she does.
This occurs even if she has emptied her bladder immediately before a workout.
She’s been working with a Pelvic Health Physiotherapist to try to increase her pelvic floor muscle strength.
Erica feels a little embarrassed that she is weak, as a Women’s Health Physio herself.
She avoids CrossFit workouts that involve jumping and feels embarrassed. Erica wonders if people know that she’s leaking when she stops and goes to the bathroom during a class.
No matter how many pelvic floor and core exercises she does it doesn’t seem to change.
She’s even tried a pessary which didn’t influence her leaking.
