About this Consult:
Rachael is a triathlete who is 22 weeks pregnant with her second child.
With her first birth she experienced Grade 3a tearing after a very long pushing phase.
She had a long recovery with stitches opening and perineal pain with sex for months afterwards, and did not feel she returned to her high intensity fitness that she loves.
She returned to HIIT circuit classes at 5 weeks post partum and ignored the cyclic symptoms she was having for some time, feeling an expectation that she should be able to return to her previous level of fitness.
She is currently experiencing symptoms related to her grade 2 cystocele and self-diagnosed pelvic girdle instability. These symptoms are aggravated by activity such as running and caring for her 2 1/2 year old son.
Rachael is frustrated by these limitations, especially as a Pelvic PT who works with clients with these concerns every day.
She is modifying her exercises, avoiding lifting, doing pelvic floor exercises and decreasing her demo role in her Pilates classes.
Rachael reports she has strong and functional pelvic floor muscle strength on self assessment, but always feels clunking in her pelvis in her Pilates activities.
For her bilateral sacroiliac joint pain, which she identifies as pelvic girdle instability, she avoids single leg activities but it came on early and painfully in the first trimester with this pregnancy, which was unexpected.
She would like to feel better at this stage of pregnancy and also return to exercise asymptomatically more quickly after this birth.
